12 Common Causes of Diarrhea Most People Don’t Know

Being stuck on the toilet with diarrhea sucks. I’ve spent a significant amount of time there. However, I’m lucky compared to my friend Jordan who had years of steady, chronic diarrhea.

The worst part of this is we aren’t alone. There are millions of people just like us who experience occasional diarrhea or even daily bouts. And sadly, it’s the world’s 2nd leading cause of death in children.

The silver lining to this unfortunate problem is that diarrhea is solvable. It’s completely fixable and not something you have to live with if you don’t want to. One of these twelve causes of diarrhea could be a stand-alone problem for you, but oftentimes the case will be more complex with several of them contributing to your daily bathroom habits. The key is that you are willing to dive deeper than modern medicine typically goes and use an engineering mindset to solve this issue. (More on this at the bottom of the post.)

Is Leaky Gut Making You Sicker?


You’ll notice that Irritable Bowel Syndrome (IBS) isn’t on this list. That’s because IBS in and of itself is not a cause of diarrhea, but the issues below are part of why your doctor might have told you you have IBS.

Our goal here is to give you a hit list that you could actually print out and take with you to your next doctor’s appointment. From here, you can have an educated discussion about troubleshooting your chronic diarrhea. This list is based on functional medicine and years of helping people struggling with this complex issue.

Gut Dysbiosis

Gut dysbiosis is a catch-all term for an imbalanced gut flora. Various well-known experts refer to it differently, as there is currently no single accepted definition of the location or the exact issue with the gut flora that this term refers to. That being said, in our digestive tract there is a delicate balance between different kinds of bacteria. If a certain species overgrows or gets wiped out, the balance can be thrown way off. Also, the whole colony of bacteria can be too small or too large and this can cause issues. One of the many possible results of this is diarrhea and sometimes persistent diarrhea. It’s very important to maintain a diverse gut flora that is at the right size (not too big or small).

SIBO – Small Intestinal Bacterial Overgrowth

The majority of the 100 trillion bacteria that are in our digestive tracts are located in the large intestine. The small intestine contains much much fewer. It is designed to be the location for food absorption. It’s not designed to harbor a lot of bacteria, but in many people who have diarrhea this can be the reason. SIBO is an overgrowth of bacteria in the the small intestine and for many who have this condition, diarrhea is the result (others have constipation). SIBO is a type of gut dysbiosis, but the difference is that there are tests and a known location of the dysbiosis.

Parasites feed off their hosts, causing harm and commonly diarrhea. It’s a major myth that people in developed areas of the world don’t get parasites. They are much more common than most people realize. Humans can pick up parasites from pets, contaminated water, food and contact with other human waste. Common parasites that cause diarrhea are Giardia, Strongyloides, Entamoeba histolytica and Cryptosporidium. It’s been our experience that a large percentage of people who have chronic diarrhea have a parasite.

Yeast Overgrowth

Candida is the most common yeast that overgrows, but any overgrowth of yeast is not good for gut health. Yeast is a fungus that is a normal part of human gut flora colonies. In healthy amounts, it actually aids in digestion and keeps us healthy. But when too much yeast is present it can start to damage the gut lining. Excess yeast can release harmful toxins that start to affect the gut wall and your bowel movements causing gas, bloating, and diarrhea.

Gluten (and Wheat in General)

Let’s be honest, gluten and wheat taste delicious. But unfortunately they might be causing you diarrhea. Gluten is a problematic protein found in wheat and has been shown to have toxic effects in our guts. It causes inflammation in the gut and can alter the structure of the gut wall leading to symptoms like diarrhea. Some people are less tolerant of gluten than others. If you have a gluten intolerance or Celiac disease it could be the culprit of your GI distress. An important point is, as the research becomes clearer, we are learning that wheat and other grains contain many other compounds beyond gluten that could cause diarrhea.

Dairy, Oh Sweet Dairy

Dairy has three components that can be common causes of diarrhea: lactose, whey and casein. Lactose intolerance happens to some people as they age. They lose the enzyme needed to break down lactose. When this happens the lactose can’t be used by the body and ends up feeding bacteria instead, which typically leads to diarrhea. Casein and whey are the two proteins in milk that can cause diarrhea for those with gut wall damage. Dairy protein intolerance is often mistaken for lactose intolerance and therefore it’s wise to cut all dairy out for a period of time if you are experiencing diarrhea.

Recommended Next Step: Truth is, not everyone needs to be dairy-free and many people like me can remove it, feel better and then re-introduce it later without issues. Here’s a short video on that process.

FODMAPs (Fermentable Oligosaccharides, Disaccharides, Monosaccharides And Polyols)

As gut health breaks down, almost any food can cause problems. But FODMAPs are especially problematic for those with damage. FODMAPs are a group of sugars/carbohydrates that depending on the health of your gut and some genetic factors can cause diarrhea. These undigested bits of food have an ability to draw extra fluids into the gut, through a process called osmosis (a fancy term for how water moves). All the extra build up of stuff in the gut is a cue for water to rush in creating a watery mixture that the body must get rid of ASAP. In our opinion, people who are sensitive to FODMAPs typically have other underlying issues that caused the FODMAP intolerance. As the other issues are resolved, the ability to digest FODMAP foods typically comes back.

Recommended Next Step: The FODMAP diet isn’t always what it appears. While it can significantly help diarrhea, not eating FODMAPs can cause health issues later on. I explain why in this article.

Emotional Stress

Did you know your gut is actually programmed to respond to stress? Have you ever been really nervous before an important event like a first date, public speaking event or competition event and had to run to the bathroom? If so, you’ve felt this phenomenon. The gut is home to the Enteric Nervous System. It’s like your 2nd brain, but only it contains way more neurotransmitters than your brain.

Anything that can excite that wonderful network of neurons in your gut can cause increased motility. Motility in your gut comes from a wave-like movement called peristalsis. This wave-like movement is controlled by special cells that generate the wave and are affected by nervous system signals. Stress has different effects on the different parts of your GI system. It  increases motility in the colon while simultaneously decreasing motility in the small intestine. This slowdown of food processing in the small intestine may lead to SIBO and damage to the mucous lining of the gut. If you are currently under constant emotional stress due to “life happening” this could be part of why you are having diarrhea.

Physical Stress

Working out is great for health. But if you are overdoing it, you could be damaging your gut. When we work out, a lot of heat builds up in the tissues of your intestines. This can lead to inflammation and irritation of the gut. For your gut the worst kind of exercise is long duration, medium to high intensity, like long distance running for marathons and triathlons.

There is also an effect where blood that was directed away from your GI system during workouts rushes back in afterward and can potentially cause further gut damage.

Both of these are physical stresses that can cause diarrhea. What’s clear is that exercise is generally more beneficial than harmful. However, part of the reason why you’re having diarrhea could be due to your exercise habits.

Inflammatory Bowel Diseases (IBD)

The two main inflammatory bowel diseases are Crohn’s and Ulcerative Colitis. As the name suggests these are diseases of chronic inflammation, which creates the conditions in the intestines for diarrhea. Food may pass through the bowels too fast or slow as a result of inflamed areas of the gut. Our experience also tells us that people with IBD typically have underlying reasons for the inflammation, which are things like infections, hormone imbalances and/or food intolerances.

Fat Malabsorption

Fat is necessary and vital for health. It is the building block of all hormones in the body. But some people, due to a variety of factors, end up in a position where their bodies can’t break down and absorb fat that is eaten and when this happens diarrhea follows. The reason isn’t always straightforward but it can be related to damage to the intestinal tract, gall bladder issues like pancreatic insufficiency and bile acid problems. The great thing is as you heal the other issues contributing to diarrhea, this problem typically resolves itself as well. But in the meantime to stop the diarrhea try paying attention to dietary fat absorption.

Traveler’s Diarrhea

Maybe it was Montezuma’s revenge, or perhaps it was that street cart you ate from in India, or maybe it was just that produce you bought fresh from the village market but didn’t quite wash enough. Anyway you slice it, Traveler’s Diarrhea is not fun. You may be thinking, “Yeah, I had it,  but that couldn’t be what’s causing my diarrhea now.” You may have overcome the virus or bacteria that caused the onset of diarrhea, but this isn’t the end of the story. Recent research is showing that just one experience with Traveler’s Diarrhea can significantly change your gut flora and a percentage of people end up developing SIBO and/or gut dysbiosis after such extreme infections. If you’ve ever gotten sick while traveling, it may be related to your current cause of diarrhea, especially if you used antibiotics to get rid of the infection.

Unravelling the Mystery of What Causes Your Diarrhea?

Even after two years into following the Specific Carbohydrate Diet, every 6 weeks or so Jordan would have a nasty couple days locked in the bathroom. People on the internet and uninformed medical professionals blamed cheating, cross-contamination and supplements for his issues.

However, when he got stool tests back that showed a gut infection of Strongyloides, well it was pretty clear what his issue was. A round of antiparasitic treatment and his seemingly “random” bouts of diarrhea were gone.

The reality is diarrhea seems undeniably complex when first learning about it. And that’s because it’s typically a combination of the factors mentioned above, as we’ve learned studying this for the past 6 years.

It varies from person to person. Jordan and I were different, and so are the 1000’s of people we’ve talked and worked with.

Once you get comfortable with the fact that there’s no one-size-fits-all prescription for people with diarrhea, then you begin to realize it’s actually easier than it’s ever been to fix it and reverse any damage it’s been doing to your body.

If this is a problem for you, even if it’s an occasional problem, I can guarantee you a few things:

  1. Diarrhea will strike when you least like it to (like dates and parties)
  2. It’s a reliable marker that your health is not anywhere as strong as you’d like to believe
  3. It’s fixable and if the right actions are taken you can trust your stomach again

    Hello. I have been reading everything I can to learn how I can stop my diareaha. I had it for months off and on and then none stop liquid for a month and was diagnosed with Celiac disease. I juiced for a few weeks and added probiotics and a gut repair powder and within weeks formed normal stools. Then adopted a strict paleo diet slowly introducing cooked vegetables and eventually salads again. I was feeling great and thought I had it licked for five months. Then bam, started with the symptoms of cramping, gas pain and lose stools. And the last three weeks nothing would stay in. Went back to juicing but that did not work this time. Came out the other end. Doctors are baffled saying my blood test for Celiac says it is in remision . So now back to square one. I see all the things above that it could be, but how does one determine if it is candida, SIBO, yeast, etc and which diet to follow? My GI doc. is clueless and will now call it IBS and giving me medicine to cover the symptoms. I can not afford a functional medicine doctor so trying to figure it out. So strange how I did so good and then flared to the point of not digesting anything for almost a month. Any insights would be appreciated!

        Does the SCD diet address Candida, ISBO, fructose malabsortion etc.? I don’t understand why the juicing isn’t working this time. I keep waiting for a stool formation before I introduce food. But its been 3 weeks. My GI is ordering stool testing but they dont look for the things above.

          I LauraBeth. The SCD diet can certainly help with those issues, it just typically takes more than dietary changes to get better if you do have any infections. I’ would wait to see what results the stool test come up with and then get on an infection protocol that correlates.

    I am in desperate need of your help. I have had dietary issues my entire life – IBS-D complications and usually very ill several days a month – but my condition has worsened terribly over the past year and a half – I have lost over 20 pounds which I could not afford to lose in the first place – I’m 5’1″ and used to weigh 110 pounds two years ago – I now weigh 89 pounds because my system has gone completely haywire and rejects everything. I don’t know who to turn to for help – I do not need to be tested for celiac because it is blatantly apparent that whenever I consume any wheat now (even baked goods made without yeast), I react to the gluten – within 12 hours, I am violently sick in the bathroom for the entire next day. I have kept a food journal for the past 2 years as my health has continued to rapidly deteriorate. I am assuming I have an autoimmune disease where my own cells are now attacking one another, foods I used to eat daily are now recognized by my body as “invaders”, and I have developed an intolerance to virtually everything. I am sure thousands of people develop gluten intolerance, but for me it is catastrophic because I am vegetarian and (now) fructose intolerant besides!! One bite of meat or poultry and I am sick within 5 minutes – so,I have lived on wheat for over 50 years – cookies, muffins, pasta, bagels, etc. – now much to my horror, I have learned that wheat is extremely inflammatory and can poke holes in the intestinal wall. But the tragic part is I have nothing left that I am able to eat and I don’t know how to survive! I tried switching to rice and that was an equally violent reaction (although I also ate rice my entire life) – at first I thought it was the arsenic content, but it was the fact that rice is also a grain. Thinking I had SIBO, I switched to white potatoes which were always fine for me – I tried 2 bites on 2 separate occasions and got violently sick from the solanine and mycotoxins…same for yams and sweet potatoes. I have since tried Rice Krispies, Cream of Rice etc. – but the horror is that I react to every grain and carbohydrate now! I then tried bananas just to keep the weight on, but in the midst of all this lunacy I also developed fructose malabsorbtion – one bite of a banana and now I double over in pain – same exact reaction to avocado (both are latex fruits…yet another new intolerance for me) – but keep in mind, I ate both of these my entire life. I tried black olives and was violently sick form the lye – although they were all natural and just packed in water and sea salt – I reacted not only to the lye, but to the fact that they are fermented…I can’t have anything fermented anymore. I had a raging case of candidia for over 8 months, so I started taking cinnamon tree bark extract thinking it would help – but at the same time, I took advice from several people who recommended I eat yogurt…4 months later my hair was falling out in clumps, the whites of my eyes were yellow, and I developed probiotic toxicity. I was so desperate at that point that I began existing on dark chocolate which is supposed to be fine with fructose issues (again I never once had a problem with fructose my entire life)…the chocolate was fine, but then I realized that it is also fermented and contains yeast by-products…and it is a prebiotic – I stopped the chocolate. I can’t eat nuts (I never could because I don’t handle insoluble fiber well) and I can’t eat vegetables for the same reason – plus now the fructose issue has made vegetables unbearable for me – even a jar of baby food strained carrots – all organic, no citric acid added – just carrots and water (2 grams) makes me sick for the entire next day. I’ve become housebound and I’m giving up on life – I need to be able to eat again. Cheeses are of course off the list because the mold strains started making me so ill it was awful – I’m even afraid to try ricotta cheese now (yes, I used to eat it all the time – along with sour cream and cheddar cheese and mozzarella cheese – no problems whatsoever with any cheese until all this insanity started) because every brand of ricotta cheese contains xanthan gum which I also react to now. This had been a journey into hell for me – even palm oil sets off a reaction – all the things I never thought twice about have become huge triggers. I used to love pasta, pizza, grilled cheese sandwiches, rice with sliced avocodo, banana cream pie, macaroni and cheese, croissants, danish pastries, cheesecake, coconut custard pie (coconut causes a violent reaction now because of the fructose content – even sulfite free shredded coconut is a nightmare I tried it 3 times and was so sick as a result). Now, I am living on custard which a local baker makes for me every day – it is costing me a fortune but the truth is that i am starving. PLEASE can this wheat and grain intolerance be reversed?? I can’t imagine going through the rest of my life without carbohydrates – I can’t eat meat, poultry, fruits, vegetables, nuts, or eggs (am severely allergic to eggs) – there is nothing left for me to eat! I am wasting away and all I want to do is sleep because I have no nutrition going into my system. Can you please help me? What is happening to me…is it parasites, bacterial overgrowth…what is it? As I said, I have had IBS for years, but I used to be able to eat normally nevertheless…, plus, IBS has over 160 forms – yes, I am celiac I guess because now I can’t eat any grains…but how did this happen “out of nowhere”…and WHY? I’m so desperate I would be happy just to be able to eat baby food rice cereal! I used to take mastic gum extract to control my IBS when I thought I had h-pylori – that worked well for me about 5 years ago, as did Holy Basil and bentonite clay and DGL-licorice…but that was then – before I even knew what being sick like this really was. I have tried to self-heal and spent thousands of hours educating myself on enzymes and intestinal conditions trying to understand why I can’t digest food anymore…but I can’t go on like this not being able to eat – the Mastica is out of the question now because it is a plant derivative and doesn’t work for me now – actually, it just makes me even more sick. Currently, I am still taking cinnamon tree bark extract (ceylon cinnamon so it doesn’t damage the liver) – but none of this even matters anyway because the fact is that I simply can’t eat anything anymore and I desperately need your help. Please – can you help me??

When we see folks who are reacting to everything and rapidly losing weight, it’s a pretty good indication that you have some underlying root causes going on like gut infections and/or hormonal issues.     Has anyone addressed Microscopic Colitis here? I have it and had a year of the steroid that is specific to MC which is Entocort. It was sort of miraculous, a real solution to my problems for nearly a year. At the same time I found a natrapath who began acupuncture and several kinds of Chinese herbs which have allowed me to come off the Entocort gently. I have given up dairy, gluten, sugar, etc. and am much better, but really, the Entocort ( from Canada) gave me the break I needed to begin healing naturally. I drink a lot of bond broth and eat fermented veggies, just got a great new juicer and am looking forward to Christmas, even though I will stick to the turkey and maybe some sweetpotatoes. I don’t mind the diet and do not crave the things I used to crave. But Microscopic Colitis is rarely addressed and can keep you from living a real life, as can any sort of colitis. It is diagnosed only with a biopsy during a colonscopy. I was actually ready to stop eating to make it stop so I am grateful for the Entocort, even though I know here all natural methods are encouraged. The steroid enabled me to be able to get the natural methods to work. Sorry for the long post, but this is a real and dehumanizing condition that many people have to deal with.

    You can also get diarrhea if you have your gallbladder removed. My Mother-In-Law had hers removed and she has to take Colestipol to control the diarrhea. It is the only prescription that she takes and she is 92 years old. It seems to help her even tho it has some side effects such as memory loss and muscle pain. I have tried to put her on bile salts and other natural things but nothing works. Merry Christmas ????

    I have had diarrhea for a little over a month now. There is a LOT of mucus in my stool and I have a localized pain on my right side. The doctors checked my liver, gallbladder, and pancreas. All is good in those areas. I’m so annoyed! My energy level is non existent, I can’t motivate myself to work out, everything I eat literally goes right through me. Now, it’s effecting my sleep. Any ideas??

    I think you should add colon cancer to your list. It’s not common but clearly very serious. Some folks reading this might just start changing diet without seeking proper medical attn first! Remember to rule out cancer people!

      Hi Jim – thanks for bringing this up! Absolutely if you’re having digestive symptoms the first thing you should do is go and see your doctor. That being said, oftentimes diarrhea is just called “IBS” and no real reason is given. This article is intended for people with Diarrhea who’ve been told by their doctor that there is nothing “wrong.”

    I have had diarrhea for three and a half week now..stomach cramps..the past couple of months has been very stressful for me my mother had a heart attack was in CCU for six weeks…she’s finally comes home same week my boyfriend has an accident at work and cuts off a finger and I don’t know if stress is causing my problem I am scared there’s something bad wrong with me now have been having panic attacks bad…can anyone help

      HI Cindy – I’m so sorry you’re going through such a stressful time. Stress definitely has an impact on the gut and we’re sure that has had at least an impact on your symptoms. We also suspect you haven’t been following your normal schedule of eating, sleeping, exercising etc. with all of this going on.

      It’s never a bad idea to talk to your doctor about what is going on!

        Thank you for your reply … I am seeing a Doctor next is I’ve never been sedated or put to sleep and I’m terrified and their wanting to do a colonoscopy I have a four and nine year old and it scares me so bad to think of anything happening and not being here for them.,

    Do you have tips or recommendations for finding a good doctor? I have seen a few different specialists over the years, but the best I’ve gotten is that my issues are caused by “stress” (which I don’t have much of) and want to perscribe medications.

    I have been battling fat malabsorption for decades & despite seeing several gastroenterologists I still do not know the exact cause never mind what to do about it. Apart from ‘fatty foods’, now also if I eat orange vegetables or veg containing carotenoids, or fat soluble vitamins I will have orange diarrhoea. With all your extensive knowledge does this give you a clue as to which of the 3 causes of fat malabsorption you mention it might be? And then what can I do about it? Can you help where 2 decades of searching & medical visits has failed. I would be eternally grateful. Yours hopefully. Best wishes Evelyn

    This is very informative! I have been suffering from diarrhea for many years now and frankly it seems like nothing helps, I’ve been gluten free for over a year or more, recently trying the AIP Paleo diet with occasionally cheats of Udi’s GF bread and muffins. I see a naturopathic and just recently found out my thyroid is underactive with Hashimoto’s apparently however the diarrhea has been a constant for years before my thyroid has changed. I avoid the foods on my food allergy test and close the AIP Paleo diet, eat very clean and healthy, changed a lot of my body products to GF as well. The only time my stool changed is when I was pregnant before our miscsrriage and when I was on antibiotics but that was the same time so it was hard to tell which one actually helped. I am so tired of having diarrhea every day multiple times a day. I feel like I am slowly dying in some aspects. Blood tests also revealed I have low vitamin D and low Ferritin. What would you suggest would be the next course of action I take with my MD?

      Hi Amber, thank you for reaching out. You certainly have a lot going on and these types of issues need a complete approach vs just one specific treatment. It sounds like you need to keep working on a specific diet plan that works for you, while letting either the SCD diet or Solving Leaky Gut diet provide the framework for you to work off of.
